All referral forms on this website are also available as eForms for OSCAR, Avaros, Indivicare, and ACCURO EMRs. You can download some of the premade forms here from WorldEMR or fill the contact form below for any other eForms that are not listed for your EMR.
Fax is inefficient, slow, and manual for sending referrals. Many clinics (including hospital divisions) are now only accepting referrals via the Ocean eReferral Network.
Ocean eReferrals can be sent directly from providers’ EMR and will auto-populate patient, specialist, and referring physician details. Patients also get email notifications on the status of their referrals sent through Ocean.
Also, Ocean now allows providers to send referrals by eFax, so referrals can still be sent through Ocean even to those clinics who aren’t yet setup for eReferral.
There is no cost for sending referrals through Ocean.
